Allied Health Professional / Case Manager Position

Following our restructure in 2025, our service model and client demand have continued to evolve. We are now looking for an experienced Allied Health Professional to join our team in a predominantly Case Management role, working with clients across Central West NSW. For the right person, there may also be the opportunity to continue some clinical work within your own allied health discipline alongside your Case Management role.

About the role

We are looking for someone who is keen to work a minimum of 24 hours /week across 4 days up to full time hours. As a Case Manager at Integrity Therapy Services, you’ll work alongside participants and their families to understand their individual circumstances, priorities, goals and support needs, coordinate complex treatment, rehabilitation, care and community supports, and bring together multidisciplinary teams to facilitate clear, purposeful communication. You’ll support participants to understand their options and be actively involved indecisions about their rehabilitation and life, build confidence, independence and self-efficacy, identify barriers and respond proactively, source appropriate services and supports, monitor progress and outcomes, and communicate effectively with iCare / Insurers, treating teams, service providers and other stakeholders.

Your work here will include:

• developing a strong understanding of each person’s goals, circumstances and support needs
• coordinating treatment, rehabilitation, care and community supports
• bringing multidisciplinary teams together and facilitating clear communication
• supporting participants to make informed decisions and build independence and self-efficacy
• identifying barriers and changing needs and responding proactively
• monitoring progress and outcomes
• communicating with iCare / Insurers, treating teams, families and service providers
• providing clear, timely and clinically reasoned documentation.

Your work here does NOT include;

• NDIS work – our service focus is now complex and catastrophic injury.
• The “return to work” push – we’re not occupational rehab. Our Case Management work is about building long-term relationships and supporting people with complex injuries over time.
• Ridiculous KPIs that leave you rushed, stressed and wondering why you chose allied health – we absolutely care about productivity and running a sustainable business, but not at the expense of quality work, good clinical reasoning or our people.

This position would be ideal for you if you have;

• At least 5 years’ clinical experience
• Experience working with people with complex or catastrophic injury, neurological conditions or significant disability
• Current iCare Case Manager approval, or the ability and willingness to apply for and obtain approval (with our support).
• Strong clinical reasoning and problem-solving skills
• The ability to build trust and work collaboratively with participants and families
• Confidence communicating and negotiating with multidisciplinary teams and multiple stakeholders
• Excellent organisation and the ability to keep the bigger picture in mind while managing the details
• Clear, professional written communication skills
• An understanding of person-centred, goal-focused rehabilitation
• The ability to work autonomously while also genuinely valuing a team approach
• A qualification such as an Occupational Therapist, Physiotherapist, Speech Pathologist, Registered Nurse, Social Worker, Psychologist, Vocational / Rehabilitation Counsellor or another suitably qualified allied health professional.
• A current Australian Drivers’ License.

As this role will provide Case Management services to iCare participants, appointment will be subject to the successful applicant being approved as an iCare Case Manager. You may already hold iCare Case Manager approval, or we will support you through the application and approval process.
