

Following our restructure in 2025, our service model and client demand have continued to evolve. We are now looking for an experienced Allied Health Professional to join our team in a predominantly Case Management role, working with clients across Central West NSW. For the right person, there may also be the opportunity to continue some clinical work within your own allied health discipline alongside your Case Management role.
About the role
We are looking for someone who is keen to work a minimum of 24 hours /week across 4 days up to full time hours. As a Case Manager at Integrity Therapy Services, you’ll work alongside participants and their families to understand their individual circumstances, priorities, goals and support needs, coordinate complex treatment, rehabilitation, care and community supports, and bring together multidisciplinary teams to facilitate clear, purposeful communication. You’ll support participants to understand their options and be actively involved indecisions about their rehabilitation and life, build confidence, independence and self-efficacy, identify barriers and respond proactively, source appropriate services and supports, monitor progress and outcomes, and communicate effectively with iCare / Insurers, treating teams, service providers and other stakeholders.

As this role will provide Case Management services to iCare participants, appointment will be subject to the successful applicant being approved as an iCare Case Manager. You may already hold iCare Case Manager approval, or we will support you through the application and approval process.
